Please read the below information to assist you in preparing for your procedure. If you require further information please refer to the admission paperwork provided by your Specialist, or contact the Practice.

Pre-operative blood test or ultrasound -
Your surgeon may request you have a pre-operative blood test or ultrasound. If applicable, please ensure this is done in a timely manner prior to your admission. Your surgeon will have provided you with the appropriate request forms, as well as informed you of the time frame required for these tests.

Mirena patients -
If you are having a Mirena in theatre please ensure you fill the script at least 48hrs prior to your scheduled theatre date, and bring the Mirena with you to the hospital.

Body wash -
The hospital may request that you purchase and use a specific body wash the morning of your surgery. Please note we do not recommend use of this body wash due to it being exceptionally harsh on your skin. The hospital is aware that Dr Leake and Dr Pontré's patients have been provided with this advice.

Medical Students -
Our Specialists are involved in the education of medical students, and as such, you may have contact with medical students throughout your admission(s).

Your permission to allow medical students to be present is appreciated, as this is the best way to encourage and develop our next generation of doctors.

If you have any objections to the above, please let our staff know early so that we may organise alternative arrangements.

Dr Leake & Dr Pontré - 

  1. Your Anaesthetist's Rooms will forward you a quote for your anaesthetic.
    • Please check with your Anaesthetist if you have questions regarding taking your medications prior to your procedure.
  2. Contact your Health Fund once you have received a quote from your Anaesthetist. You will be asked to provide the following information, all of which can be found on the quotes provided by your Specialist and Anaesthetist:
    • Surgeon name
    • Item number(s)
    • Surgeon cost(s)
    • Anaesthetist name
    • Anaesthetist cost(s)
    • Name of hospital
  3. Please confirm your surgeon's out of pocket or gap costs with your Health Fund.
    • Once you have understood this information, please sign your quote. In doing so, you are providing 'Financial Consent'. If applicable, your credit card details will be requested and are to be provided on this form.
    • Return your Financial Consent to our Rooms as early as possible. We must obtain your Financial Consent prior to your procedure.
    • Contact our Rooms no later than 7 days prior to your procedure to arrange payment of any gap amounts, as shown in your quote.
  4. Complete your online hospital admission here. If you have difficulties accessing this site or completing the required information, please call Hollywood Private Hospital on (08) 9346 6456.

  5. If you are staying overnight, you will be required to complete a pre-operative blood request. This is to be completed 2-3 days prior to your procedure and will be provided to you by our Rooms.
